Easier Reporting of Anaesthetic Incidents – Workshops 2010

The Anaesthetic eForm offers a portal to report any anaesthesia related untoward incidents for review and learning. It is available in England and Wales as of Monday 30th November 2009.

The eForm has been collaboratively developed and piloted by the National Patient Safety Agency, Royal College of Anaesthetists and Association of Anaesthetists Great Britain and Ireland. SALG will be involved in reviewing anonymised reports on high priority incidents in liaison with the NPSA and the analysis of all anaesthesia related incidents on a regular basis. The aim is to ensure that appropriate lessons are shared across the profession and that practice changes accordingly. SALG will be involved in the development of safety notifications in response to patient safety issues and themes emerging from the data.

The eForm is an integral part of Patient Safety Direct, a programme of work that aims to make incident reporting easier and engage clinicians in using the data for the benefit of local and national learning. This initiative has been acknowledged by the House of Commons Health Select Committee and was mentioned in their 2009 report Patient Safety.

A series of workshops in the first part of 2010 provides the opportunity to find out more about the development of specialty specific reporting, demonstrate completion of the eForm and the subsequent process of review and determining feedback.
