Patients with a hip fracture should ideally have surgical treatment as soon as possible, or within 36 hours from admission.¹⁴

Continuity of care should be a priority in prolonged cases and when this is not possible, a formal documented process with some overlap should be in place for handover of clinical care from one anaesthetist to another.³

A subgroup of patients with chronic thromboembolic pulmonary hypertension (CTEPH) will benefit from surgery and should be managed in designated national centres. Currently only one UK centre provides specialist surgical intervention for patients with CTEPH.

Local anaesthetic solutions intended for epidural infusion should be stored separately from intravenous infusion solutions to minimise the risk of accidental intravenous administration of such drugs.⁶⁴

Anaesthesia records from their previous care should be available to the new service (or a summary document should be provided).³³

Online imaging results from referring hospitals and within the neuroscience centre should be available locally, and consideration should be given to the provision of remote access for all anaesthetists who provide cover to neuroanaesthesia out of hours.

Staff assigned to the role of anaesthetic assistant should not have any other duties that would prevent them from providing dedicated assistance to the anaesthetist during anaesthesia.³⁹

Exposure to ionising radiation should be kept to a minimum using screens and personal protective equipment such as lead gowns and thyroid shields. Remote secondary monitors in screened viewing areas should be provided and staff should remain as distant from the imaging source as possible if they remain in the x-ray environment.^56,57

When members of the healthcare team are involved in a critical incident, they can be profoundly affected. A team debriefing should take place immediately after a significant critical incident. The lead clinician should review the clinical commitments of the staff concerned promptly. Further practical and psychological support may be necessary to assist individuals to recover from a traumatic event.²⁵

Children currently transition to adult congenital heart disease services at the age of 16–18 years, although transition services are integrated into the care pathway from 12 years of age. Anaesthetists should be aware of legislation and good practice guidance relevant to young and vulnerable adults.^35,41
