Midwifery care of a pregnant woman receiving regional analgesia in labour should comply with local guidelines that have been agreed with the anaesthetic department. Local guidelines should include required competencies, maintenance of those competencies and frequency of training. If the level of midwifery staffing is considered inadequate, regional analgesia should not be provided.165

All guidelines should be dated and regularly reviewed. All guidelines should have a clearly documented author and review date and be published in line with local clinical governance policies with appropriate oversight.

Multimodal analgesia should be used, including regional anaesthesia. Opioid analgesia can be used if required, but the patient should be given advice regarding observing the infant for signs of excessive drowsiness.
